Physical Therapist
JOB SUMMARY
The staff Physical Therapist is responsible for initial evaluation and establishment of the
treatment plan and goals in coordination with the referring physician of patients of all ages served:
neonates, pediatrics, young adults, adults, and geriatrics.
The Physical Therapist performs physical therapy treatment, administers physical therapy
modalities and therapeutic exercise, and provides patients and their families with education.
The Physical Therapist supervises and delegates tasks to the Physical Therapist Assistant. The
Physical Therapists will participate in team conferences/care planning. The Physical Therapist maintains
effective communication with referring physicians. The Physical Therapist may serve as a clinical
instructor of Physical Therapist/Physical Therapist Assistant students.

Physical Demands: Must be able to see, hear, read, write, and communicate effectively;
prolonged, extensive or considerable standing and walking; frequently lifts, positions, pushes,
and/or transfers patients and equipment; must manipulate small objects with dexterity; frequently
required to stoop, kneel, crouch, and crawl; and occasionally lift more than 100 pounds.

QUALIFICATIONS
A. Education: Physical Therapist must be a graduate of an AMA-APTA approved school of physical
therapy with a Bachelor’s, Master’s, or Doctorate of Physical Therapy or equivalent in Physical
Therapy.
B. Licensure: Must have an active license for the state of Illinois or a permit from the licensing board
awaiting licensure.
