Case Manager RN - Case Management
Job Summary: The primary focus of the Case Manager Registered Nurse is to ensure optimal patient outcomes by providing excellent quality of care and efficient resource utilization and reimbursement for services. The Case Manager is responsible for navigating the continuum of care across all disciplines efficiently to enhance the patient’s overall health and wellbeing, reduce illness, provide individualized care, and, in turn, prevent readmissions to the hospital.
Qualifications:
A. Education: Graduation from an accredited school of nursing. Current registration with the Illinois Department of Professional Regulation. Minimum three years of Med-Surg experience as a Registered Nurse.
B. Training and Experience: Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS)
C. Job Knowledge: Care planning, coordination, and care settings. Knowledge of community-based resources. Basic knowledge of payor source requirements and guidelines, including benefit coordination, authorization management, utilization review, insurance approval, and appeals processes.
Comprehensive knowledge of general nursing theory and practice, including biological, physical, social, and medical sciences and their application. Familiarity with the organization and function of all departments; familiarity with the Nursing Department's standards, policies, and procedures. Knowledge of literature and new developments in the nursing field.
Professionalism, organizational skills, and the ability to work in a self-directed manner and make decisions independently.
C. Job Requirements/Essential Functions: Able to see, hear, read, write, talk, and communicate effectively. Detect odors, stand for extended periods, walk, sit, squat, stoop, bend, crawl, lift up to 50 pounds, reach overhead, push, pull, manipulate small objects with dexterity. Critically think, practice autonomously, and address departmental needs as necessary.
